File Icons Missing

    Last week, Windows Update indicated an Important update, it was SP2. I
    installed it. After the reboot, all of my "Firefox Document" (not the
    program launch Icon, which is fine) Icons reverted to the plain folder
    icon; all other icons are OK.

    I deleted the IconCache files for both my userid and Administrator, and
    rebooted, to no effect. I changed from 32 bit to 16 bit screen colors,
    rebooted and back again with no effect. Finally, I uninstalled firefox
    and reinstalled. This restored the icons but only until the next boot,
    when they reverted to the vanilla folder icon.

    If I change the default browser to IE, all the html/htm icons display
    the proper IE Icon, but when I make firefox the default browser again,
    they become plain again. This icon issue is visible in both explorer
    while browsing folders, and on the desktop display.

    Others have had the same issue, but to date no one I know of has found a
    fix.

  2. Did you check file extension association with that application, which used
    to be when icons were displayed properly?
    Once you disassociate for example JPG files from e.g. MS Paint, and
    associate with another application, all JPG icons change.
    Similarly your Firefox may've become disassociated from htm/rtc wenpages
    files, or whatver that Firefox doc was...

    Yes I did. In fact when I look at properties, the file is correctly
    associated with FF, and the correct icon appears in the association box.
