In another thread DSTM recommended that you install Speccy http://www.piriform.com/speccy/download to check your temps. Did you do that and, if not it is still a good idea to install the free version and see what it says about your GPU temp. That being said, you are probably right and it time for a new card. Probably is not definitely so check the temps and see if that gives rise to a solvable issue.

Most prob your card but I would try other options first.

The image in post one points normally to heat or drivers.

A stuffed Ram stick I have seen cause similar. Try 1 ram stick at a time.
Corrupted drivers can cause this on boot.
Clean contacts and reseat your card.
    Check for new drivers here

Try and get a reading on your temps
If all this fails then it has to be the card. The card could have been overheated too many times and finally gave up.
Try the card in another computer and see what gives or try and borrow a card.

    Has this mobo onboard graphics?
If it has remove the card and hook your monitor up to the mobo graphics port. It will be VGA so you will need the cable (normally blue ends on the cable.)
You may have a failing Hard Drive.
Any beeps on boot, clicking or grinding noises?
