How to clean install Windows 10

Windows 10 has been available for a couple of days, and 14 million machines have already been upgraded. If you're anything like me and you have a custom PC or you just want a nice and clean install of Windows then you'll want to format and start over. Microsoft hasn't been exactly clear about that process, but I've spent the past couple of days figuring out what does and doesn't work.
