This error can mean multiple problems. You could have bad memory or a bad memory controller on the motherboard. The cpu could be dying or the motherboard itself might be dying. This could be a corrupt monitor driver. Try removing the monitor while in safe mode and then on reboot let it re-install the monitor.

A known cause of this error is bad capacitors on the motherboard.Do an inspection for any that might be bloated, cracked or leaking.

This can also be caused by a driver installed by a third party app such as those that install when you use wireless device from a mobile carrier. Some games also install a driver and this could be the problem. It could be your ant-virus causing the problem.

In summary and with the problem still being present even after a clean format and re-install my bet is bad motherboard or memory controller.

Motherboard looks ok from what I can tell (not too experienced at looking for faulty parts). I got rid of the old monitor driver, just let windows install one, guess I will try that out.
